Open Enrollment is Monday, November 4, 2024, through midnight on Friday, November 22, 2024. It’s an excellent time to make decisions that will impact your health and wellness for the year ahead. Selecting the right medical plan is more than just coverage—it's investing in your overall well-being. ShopRite is empowering you to make healthy choices by providing medical plans that support both your current health needs and your long-term wellness goals. The right plan can improve your quality of life by offering the services and care you need when you need them. From preventive care to treatment for chronic conditions, our medical benefits offer flexibility for every stage of your health journey.

WELL-BEING PROGRAMS
An important part of your medical plan is improving long-term health and managing the cost for both our associates and ShopRite. That’s why we provide programs and resources that encourage healthy actions at no additional cost to you.

OPTIONAL LIFE INSURANCE
Optional Life Insurance and AD&D are offered on an Associate
Paid Basis.

LONG-TERM DISABILITY
Benefits that provide a portion of your salary in the event you become disabled and cannot work.

MEDICAL
Your medical plans are offered through Aetna and in-network preventative care is covered at 100%

NETWORKS
For non-union full-time associates, when you enroll for medical coverage, you also get to select one of two networks.

PRESCRIPTION DRUGS
When you enroll in one of the medical plans, you also receive prescription drug coverage.

VISION
Benefits to help pay for eye exams and glasses/contacts.

Open Enrollment is your annual opportunity to review your current benefit plans and make any changes for the upcoming plan year. Here's what you need to know for 2025.

NEW DENTAL PROVIDER
Aetna is the new dental provider for 2025. You will receive the same level of care through Aetna with no changes to your plan design. However, it does offer an extensive network which means more cost savings for you.

NEW LIFE INSURANCE PROVIDER
MetLife is the new life insurance and accidental death and dismemberment (AD&D) provider for 2025. Your current Basic and Optional enrollments will be carried over with MetLife.

NEW LONG-TERM DISABILITY PROVIDER
MetLife is the new long-term disability provider for 2025. If you chose an after-tax election during the current plan year, this will carry over automatically.

THE ACO OPTION RETURNS FOR 2025
This is a group of healthcare providers that provide coordinated care to patient populations, including incentives to improve the quality of patient care and health outcomes while controlling costs.

Consider the following before you enroll.

Understand your benefit options

Complete the enrollment forms by November 22, 2024, if you want to:

Enroll in, change, or drop your medical, dental, and/or vision coverage (if you elect anything other than single coverage; identify all eligible dependent(s) on the enrollment forms).
Participate in a Flexible Spending Account (sign up for direct deposit/electronic funds transfer (EFT) for faster reimbursement by visiting www.inspirafinancial.com).

If you do not elect to make any changes by November 22, 2024:

Your medical, dental, and/or vision coverage will continue with the 2025 plans and contributions.
Your dependents will remain the same.
Your participation in the Flexible Spending Account(s) will end on December 31, 2024.

Need to Make a Change After Open Enrollment?
Once Open Enrollment ends, you may only make changes to your benefits for a Qualified Life Event (e.g., marriage, divorce, birth/adoption of a child, or loss of coverage). Otherwise, your next opportunity to make changes will be during the next Open Enrollment period, which will be held in the Fall of 2025 for coverage in 2026.

Download your enrollment forms below and sign up for 2025 coverage. The enrollment form includes your weekly contributions. Contact your Benefits Administrator, if you require assistance with accessing your enrollment form.

Review your enrollment materials and Summary of Benefits and Coverage (SBCs) on your benefits website.
Consider your benefit needs and compare your options for 2025.
Contact your Benefits Administrator if you have any questions.

Make your benefit decisions

Choose your benefits carefully.
Decide if you want to add or delete dependents from your coverage. If you’re adding a dependent, you are required to provide dependent documentation, such as a marriage certificate and/or a birth certificate.
Review your life insurance needs and update your beneficiary designation, if changes are necessary.
Consider making pre-tax contributions to the Health Care and/or Dependent Care Flexible Spending Account(s) (FSA).
Review your LTD needs and update your tax election, if changes are necessary.

Sign up for 2025 Medical, Dental or Vision Coverage Decide who will be covered. If needed, provide Marriage License and/or Child(ren) Birth Certificate(s).	Complete a benefit enrollment form.
Waive your ShopRite medical coverage.	Complete the waiver form.
Waive your ShopRite dental or vision coverage.	Complete the waiver form.
Optional Flexible Spending (FSA) Health Care or Dependent Care.	Select the FSA option on the benefit enrollment form.
Long Term Disability Benefits (LTD) - How do you want to receive your benefit?	Complete the LTD enrollment form.
Basic Life Insurance	Complete the basic life insurance enrollment form.
Optional Life Insurance	Complete the optional life insurance enrollment form.
